Exams to Change
In the area below please enter the exams/tests you wish changed in the format of Course Name and Number and CRN number if possible. (eg. BIOL2440, CRN 655432). Indicate if the change affects Term Tests/Midterms; Finals; or ALL exams for term. One exam per line would be preferred.

Reason for Change


There is a $50 fee to change your exam site for each exam scheduled in Memorial Self-Service. This fee will be charged to your MUN account if your request is approved. To avoid incurring fees in changing your exam site, please submit the change of exam site form during the first week of the semester.

Requests for changes in exam sites must be submitted on this form to Distance Education, Learning and Teaching Support, at least 14 days prior to the exam date. Changing your address does not automatically result in your exam site being changed; an official application must be submitted. We reserve the right to refuse any requests for changes in exam sites.

If you are transferring your exam to a location outside Newfoundland and Labrador, you must complete an Examination Supervisor Form.
